If you are unable to make it to our office for an initial consultation, we have a program where you can establish a remote patient chart. You can order the vials from our online store with your specific chart number from our online store.

In order to establish your membership chart , we need the following:

1. Upon purchase of this item, you will be able to download the appropriate forms to start your chart
2. Complete and return these forms along with the bullet point items listed below to Alexis@Delightmedical.com.
3. Please call the office to confirm reciept if you have not recieved an email confirmation within 3 days.
4. Once you have confirmation from our office, then you can start ordering our injectable vials.
5. Obtain and provide us with the following Data:

• Current weight, height, and measurements of your neck, abdominal circumference, and hip.
• Blood pressure and heart rate.
• Head-to-toe photo.  (you may blur your face if concerned about privacy)
• Your Latest Blood test results.
• Another option is to send us a copy of your doctor’s notes from a recent visit including your medical history, blood pressure, heart rate, weight & height, and a copy of your most recent blood test.

Once we are in receipt of the above items, you will then be assigned a chart number that will enable you to take advantage of excellent weight management tools on our Web site which are limited to established remote patients only. Once your chart is established, you will be able to start your purchase of any products, injections, physician phone consults, or specialized diagnostic tests that are available on our special remote patient VIP section of the online store!
